Health cover in Spain

So you’re moving to Spain, and you’ve found your dream Spanish property. But what about living in Spain? One thing is certain – you will need to have health cover in one form or another.

E121 and E106

If you are of pensionable age you should obtain a form E121 from the DHSS. Having obtained a residence permit this entitles you to obtain a health card and receive permanent state health care without the need to renew.
An E121 can also be applied for if you are unfit for work and claim incapacity benefit. If you are taking early retirement or seeking employment you can apply for a form E106. This can only be obtained if you have worked and paid taxes in the UK for the previous three years. It is valid for two years and cannot be extended.  After this time it is assumed you have either reached retirement age or found work and are therefore covered by Spanish social security system. Dependants of a holder of either an E121 or E106 can be added to their cover.

BUPA

After two years you will be expected to take out private medical insurance if you are not covered by the Spanish state system. Holidaymakers, or those with a holiday home in Spain, should obtain an E111 form which is valid for one year and renewable for a further year. This entitles you to routine emergency medical treatment. After this period you will be considered to be living in Spain and expected to either apply for an E106 or E121, as appropriate, take out private health insurance, or join the Spanish social security system.
